Recently listed by the U.S. Department of Health and Human Services Agency for Healthcare Research and Quality (AHRQ), this is the first PSO solely dedicated to using healthcare debriefing practices to enhance safety and quality improvement.

PSOs, established under the Patient Safety and Quality Improvement Act of 2005, facilitate the confidential and voluntary reporting of quality and patient safety data by individual healthcare providers and organizations, offering protection against legal discovery. Through this mechanism, PSOs assist healthcare professionals in analyzing quality and patient safety issues to proactively prevent future occurrences. The AHDQ PSO will support healthcare organizations by collecting and analyzing debriefing data from healthcare teams. In turn, the AHDQ PSO will provide confidential feedback that helps teams and organizations to foster better debriefing practices, enhance psychological safety culture, reinforce joy in work, and minimize harm to patients and teams.
